Multiple Worksite Report
The Multiple Worksite Report (MWR) is a quarterly survey of large employers used to collect statistical information to supplement statewide data provided by employers through State Unemployment Insurance Quarterly Contribution Reports. All information collected by the Multiple Worksite Report program is kept strictly confidential by the Bureau of Labor Statistics. The various state agencies cooperating in the MWR program use this data for statistical and Unemployment Insurance program purposes and hold the data confidential to the extent allowed under their respective state laws.
 
This survey is conducted on a quarterly basis by state. Most employers with more than one location and 10 or more employees combined in their secondary locations are required or requested to complete the MWR.
The Bureau of Labor Statistics gives the employer an option of filing the Multiple Worksite Report by electronic data reporting or by magnetic media. The Electronic Data Interchange Center in Chicago, Illinois, will take the reported data and send the required information to state agencies on an employer's behalf. Contact your payroll service to find out if electronic data reporting or magnetic media is an option that is provided by your payroll software or payroll service.
